History

• Right sided headache, visual scotoma, and left arm paresthesias one week prior to admission.
• Episodes of left hemparesis and dyphasia during examination
• Anticoagulated, but episodes of dysphasia and left hemiparesis continued
• CT brain normal
• MRI brain showed small bilateral infarcts
• Examination on arrival in Phoenix - normal
