Case 4 History
Presented by: Dr. Robert Griebel, Neurosurgeon, Saskatoon
Age: 40
Sex: Male
History:
Sept 23/99 - Work related injury. Severe LBP with radiculopathy unresponsive to physio or NSAIDs.
Jan 5/00 - CT scan L4/5 disc bulge. Conservative Rx continued as did pain.
Mar 7/00 - MRI- L4/5 disc bulge with associated small canal.
May 00 - Severe exacerbation of pain - unable to stand or walk. Pain now in both legs. MRI repeated but no substantive change.
June 14/00 - Surgical procedure - Hemilaminectomy and excision of disc material. Pain recurred after several days post-op.
June 22/00 - Repeat exploration of nerve root. Little of note. Biopseid tissue grew a colony of pepotostreptococcus. Pt given 6 week course of clindamycin. No change in condition.
