History: Rheumatoid arthritis x 16 years. Neck pain x 3 months.
Examination:

  • External joint deformaties.
  • Neck movement has become uncomfortable.
  • Grade 4 power in all limbs.

Pre-op Xrays
Lateral C-spine xrays in Flexicon, Neutral and extension showing C1-2 instability.

Treatment: What would you do surgically?
