Hangman's Fracture

Case 3

Age: 79

Sex: Female

History: March 1, 2001 - Fell at home. Complained of neck pain. Neurologically intact. Sent in with a hard collar.


Pre-op X-Rays

Pre-op Lateral C-spine X-Ray showing C2-3 dislocation. Note the abnormal spino-laminal line.


Open mouth view of c-spine showing distorted odontoid.


CT scan of cervical spine showing fracture through the body of C2.


Treatment:

Patient was treated in a halo traction and vest. She will be monitored radiologically every 2 weeks.


Post-op X-Rays

Post-op Lateral C-spine X-Rays showing reduction of the hangman's fracture. Note the realignment of the spino-laminal line.


Comments:

- Hangman's fractures are usually caused by extension injuries.

- They usually heal with halo vests. Very few require surgical treatment.
