Case 4

Age: 80

Sex: Male

History: Headaches, right periorbital pain, ataxia, double vision, poor vision on right side x 6 months. Past history of CA prostrateExamination showed right ptosis, blurred vision on right side and ataxia of gait.


Pre-op Xrays

MRI of brain with no contrast showing right suprasellar mass


MRI of brain with contrast showing 2 evenly enhanced masses. The 1st one is right suprasellar and the 2nd is in the left cerebellum.


Clinical Diagnosis:

  • Multiple Menengiomas
  • Metastasis

Treatment options:

  • Leave alone & repeat MRI in one year?
  • Surgery?
  • Surgery followed by stereotaxic radiation?
  •  Stereotaxic radiation followed by surgery?
  • Stereotaxic radiation?

Dr. Fred Gentili, Neurosurgeon, Toronto

Patient seen in Regina by Dr. Gentili. He feels that the perionbital pain is unusual for meningioma. Differential should include metastasis. Suggest biopsy.
