History: February 1, 2004 - Left facial pain 5 years ago. Resolved with Tegritol. Was off Tegritol 5 months ago. Pain returned shortly after. Now associated with diziness and double vision.

Examination

- Alert. Orientated

- Left 5th and 6th palsies

- No long tract signs

Impression: Meningioma Left Petrous Temporal
