Case 22
Age: 42
Sex: Female
History: She presented in 1996 with a subarachnoid haemorrhage due to ruptured left ICA aneurysm at the level of the ophthalmic artery. I did a craniotomy and clipping of the aneurysm then. She recovered completely. She now presents with one week of headaches and visual field defect. No haemorrhage on CT.

Question
I think the clip has definitely slipped and she now has a giant aneurysm. I think she would be a good case for either surgery or coiling. If you agree that she is an acceptable case for coiling, then I would send her for coiling. What do you think?
