Middle Cerebral Artery Aneurysm, R

Case 2

Age: 61

Sex: Male

History: Sudden severe headache. Presented three days later after another episode.


Examination:

Showed him to be alert but confused. CT confirmed SAH.

Pre-op angiogram showing right middle cerebral artery aneurysm.


Treatment

Arrangements were made for surgery. He was placed on Nemodipme. BP maintained at 120-160 systolic. He suddenly deteriorated on the morning before surgery. GCS dropped from 14 to 8. He was intubated and CT done.


Pre-op CT2

CT of head showing re-bleed and right frontal haematoma and intraventricular haemorrhage.


Treatment 2:

Emergency Craniotory, evacuation of haematoma and clipping of right MCA Aneurysm was done. His GCS improved to 13 post-op.
