Case 11

Age: 26

Sex: Female

History: Sudden severe headaches and drowsiness and vomiting on March 2, 2001. Examination showed GCS of 14. Truneal ataxia.


Pre-op Images

Pre-op CT head showing right intracerebellar haematoma and no hydrocephalus.


Pre-op vertebral angiogram showing right AVM.


Treatment:

Emergency surgery and evacuation of hematoma and excision of AVM. Slightly ataxic post op. GCS 15.


Post-op Images

Left CT head showing no haematoma.


Pre-op vertebral angiogram showing no AVM.


Comments:

  • Where was the AVM?
  • There was a 1 hour delay for the angiogram. Would any surgeon have proceeded without an angiogram?
  • This case is different from case 14 in that this one was drowsy and vomiting. Would anyone have done emergency embolizationbefore surgery?
